The Pediatric Palliative Care Coalition (PPCC) is pleased to provide the UPDATED PPCC Care Plan Book created to help those caring for children, adolescents or teens with medical complexity. Recognizing how challenging and time consuming managing all the tasks, appointments and information can be, our goal is that PPCC Care Plan Book will become a valuable resource for families, caregivers and medical professionals involved in this journey.

Below you will find the Care Plan Book divided into its chapters. You may download these free of charge including multiple copies of pages that are most useful. The PPCC Care Plan Book is also available to order. (1) PPCC Care Plan can be ordered free of charge by families or caregivers.
